One of the major current challenges in maintaining methadone maintenance therapy is the survival rate of treatment or prevention of recurrence. The aim of this study was to determine the effective factors on addiction recurrence in patients undergoing methadone maintenance treatment in Hamedan addiction treatment centers. This descriptive cross-sectional study was performed on 384 patients undergoing methadone maintenance treatment in Hamadan addiction treatment centers in spring and summer of 1395. Sampling was done in a proportional classification. The data collection tool was a researcher-designed questionnaire and demographic variables and the questionnaires were completed by self-report and interview. Data analysis was done using SPSS software (version 19). The rate of recurrence in patients treated with methadone maintenance was 48.2%. Temptation, friends' pressure, depression and lack of methadone dosage were among the most important causes of recurrence. According to the results, it seems that friend’s temptation and peer's pressure are the most important reasons for recurrence of addiction.
